Routine Assessments:
Our assessments are tailor-made to meet the client’s needs. These assessments may include blood pressure checks, heart and lung sounds, weight, edema and pulse oximetry. With thorough weekly assessments we can spot the early warning signs of health problems. We will contact your doctor with our findings if we detect a change in the client’s condition. Based on the doctor’s recommendations we will adjust the plan of care as needed. Some changes in condition require only a minor medication adjustment. Early intervention in health problems can decrease the need for hospitalizations or the need to move people from their place of residence. We can change foley and suprapubic catheters in the client’s residence. We see people in their homes, apartments, assisted living and skilled nursing facilities. With weekly visits the client receives 24 hour on-call availability by our nurses.
Medication Management:
Taking medications correctly is an important way to prevent problems that may lead to hospitalizations. During weekly visits the nurse will pick up the client’s prescription refills and any over the counter medications they need. The nurse will fill a pill box or electronic medication reminder with the medications needed for the week. This will allow the non-medical caregivers and family members to accurately remind the client to take their medications. If any refills are needed we will request them from the pharmacys. We fill insulin syringes. We assess the client’s response to the medications and report problems to their physician.
Instruction:
We can instruct the client and caregivers how to more effectively follow doctor’s orders and recommendations. This can include specialized diets, fluid intake, equipment use, blood glucose meters, oxygen, safety measures and preventing falls.
