The people we care for have chronic diseases that put them at risk for frequent hospitalizations. We see people with cancer, heart failure, high blood pressure, diabetes, chronic obstructive pulmonary disease (COPD), chronic kidney disease (CKD), Parkinson’s, Alzheimer’s, Multiple Sclerosis (MS), neurogenic bladder and mental health issues.

Our services are especially helpful for people who have no family living nearby. We keep in contact with relatives, case managers or anyone else the client would like to have included by emailing our weekly visit notes. Good communication means fewer surprises when problems arise. We follow HIPPA guidelines to protect the client’s privacy.
