Jan Homan, RN, BSN - Owner
Jan Homan is a Registered Nurse in Milwaukee, Wisconsin. She graduated from Avila University with a Bachelor of Science in Nursing. Her entire nursing career has been in Home Health Care. In 2000 she founded Lakeview Nurse Associates, LLC in order to provide chronic disease management to clients in their place of residence.

Jan has given group presentations on the subjects of medication management, preventing hospitalizations, improving quality of life for the frail elderly and how to implement palliative care in the home. She also conducts inservice training for caregivers in facilities for the elderly.

Jan is the past president of Continuing Care Coordinators, a Milwaukee area networking and educational group for professionals providing health care to seniors. She is also a member of Wisconsin Nurses Association, Home Health Care Nurses and Sigma Theta Tau.

The services provided by Lakeview Nurse Associates are not covered by Medicare, Medicaid or private insurance. These services may be covered by long term care insurance. Rates are per visit and include documentation, phone calls, conferencing, travel and 24-hour on-call. The frequency of visits is determined by the client and family.
