Lakeview Nurse Associates, LLC is a home health care company providing chronic disease and medication management. We help older adults remain independent and reduce the frequency of hospitalizations through routine assessments and coordination of medical care. Lakeview Nurse Associates, LLC is locally owned by Jan Homan, RN since 2000.

We are Registered Nurses who make routine home visits for skilled nursing care. We work alongside families, caregivers, physicians, social workers and therapists to develop a plan of care. The goal of this plan is to keep the client in the place and level of care preferred. Routine assessments during weekly visits help to decrease the number of hospitalizations and visits to the Emergency Department.
